The Adult Day Services program provides functional skills training activities for adults with physical disabilities, cognitive conditions, sensory disorders and neurological impairments, with the goal of helping each person be as independent as possible.
Our team is ready to help from day one. We work with client families and their care teams to develop an individualized program plan (IPP), which sets goals to maximize each person’s level of independence, with a strong emphasis on developing important daily life skills. Engaging, interactive and fun classes are offered at our Green Bay, Kimberly and Two Rivers locations and are designed to help clients meet goals, explore interests and challenge themselves in a safe and supportive environment.
CP Adult Day Services is contracted with several Managed Care Organizations, including
Lakeland Care,
Community Care,
Inclusa and
IRIS. Together, we develop a plan that provides the best experience and meets the need of every client.

In addition, CP works with area school districts to help transition people from a high school setting to an appropriate day program, like CP. Ask your school district’s Special Education team or Transition Coordinator about connecting with CP.
